Appears to be a variant of Solidisk UVIPROM 1.1 (http://bbc.nvg.org/rom/SoliDisk/util/UVIPROM-1.1.rom)

Found in VectorEyes's collection of stuff: this EPROM, that apparently came with an EPROM programmer looking like this one: http://8bs.com/see/multprm.jpg - and these two items did seem to work together, though it wasn't 100% reliable. I'm assuming this is age-related problems with the hardware.

The differences are all in the text messages. (< lines are from the bbc.nvg.org copy, > lines are from this EPROM.)

Maybe this was a rebrand, done without the original programmer's involvement, of which few were sold?
