That is an Ether1 card, it doesn't have a ROM on board.
It also looks like you're running the Universal boot that loads the Internet5 stack. Ether1 isn't compatible with Internet5, only Internet4, so you're going to need to swap some things about to get it to work.

The Starfighter 3000 demo had a different level on it to the released game, I believe. (Admittedly there's about 90 levels in the full game and I'm not sure I've played them all).

I'll pack my bits for imaging harddiscs, I can do IDE and SCSI easily ... theoretically I might be able to do ST506 (I do have the ST506 podule and can hack the software to do it) but I've no discs to test it against
